CREDIT AUTHORIZATION AND VERIFICATION DISCLOSURE
By submitting this application, you authorize Jemez Valley Credit Union (“the Credit Union”) to obtain consumer reports from one or more consumer reporting agencies in connection with your request for credit. You also authorize the Credit Union to verify employment, income, and any other information necessary to determine your eligibility for credit.
If credit is granted, you authorize the Credit Union to obtain additional consumer reports for the purpose of reviewing, updating, renewing, extending, or collecting on your account.
You understand that information obtained may include, but is not limited to, credit history, employment history, income verification, and other data bearing on your creditworthiness, credit standing, credit capacity, character, general reputation, personal characteristics, or mode of living.
You certify that the information provided in this application is true and complete. You acknowledge that this authorization shall remain valid in connection with this application and any resulting account or loan.
By checking the box or signing electronically below, you agree that this authorization and your signature may be provided and retained electronically in accordance with the Electronic Signatures in Global and National Commerce Act (E-SIGN Act).

Jemez Valley Credit Union – Online Loan Application Terms & Conditions

1. Purpose and Acceptance
These Terms and Conditions (“Terms”) govern your use of the Jemez Valley Credit Union Online Loan Application Platform, provided through VisiFI’s Digital Banking Platform (“Platform”). By accessing or submitting an application through this Platform, you agree to these Terms and consent to receive records electronically under the Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. §7001 et seq.).
In these Terms, “Credit Union,” “we,” “us,” or “our” means Jemez Valley Credit Union; “you” and “your” refer to the applicant(s). The Platform technology is operated by VisiFI, Inc. as a service provider to the Credit Union.
2. Acknowledgment of Required Disclosures
As part of this application process, you have been provided separate disclosures explaining your rights and our responsibilities under federal law, including:
- The Credit Authorization Disclosure (Fair Credit Reporting Act / FACTA); and
- The Equal Credit Opportunity Act (ECOA) Notice (Regulation B).
By continuing, you acknowledge that you have reviewed and agreed to those disclosures.
3. Electronic Records and Signatures (E-SIGN Consent)
You consent to the use of electronic signatures and records for this transaction. You may withdraw consent at any time by contacting us; doing so will not affect the validity of prior electronic documents. You confirm that you can access and retain these electronic records.
4. Privacy and Data Security
Your information is collected and used only as permitted by law and our Privacy Policy, available at https://www.jvcu.org/news-and-resources/privacy-policy.
Data transmitted through this Platform is encrypted using industry-standard protocols (SSL/TLS). While VisiFI, Inc. provides secure hosting and transmission services, Jemez Valley Credit Union remains responsible for protecting your member information and ensuring compliance with NCUA Part 748 and applicable privacy laws.
5. Use of the Platform
You agree to use the Platform solely for the purpose of submitting and managing your loan application. You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. You may not use the Platform for any unlawful or unauthorized purpose, nor attempt to modify, reverse-engineer, or interfere with its operation.
All intellectual-property rights in the Platform are owned by VisiFI, Inc. and licensed for use by Jemez Valley Credit Union. These rights do not transfer to you through use of the Platform.
6. Indemnification and Limitation of Liability
To the fullest extent permitted by law, you agree to indemnify and hold Jemez Valley Credit Union and VisiFI, Inc. harmless from losses or damages arising out of your misuse of the Platform.
The Platform is provided “as is” and “as available,” without warranties of any kind, express or implied, including merchantability or fitness for a particular purpose. Neither the Credit Union nor VisiFI shall be liable for any direct, indirect, or consequential damages arising from use of or inability to use the Platform, even if advised of such possibility.
7. Eligibility
This Platform is intended for U.S. residents aged 18 or older who are eligible for membership with Jemez Valley Credit Union.
8. Changes to Terms
We may update these Terms at any time by posting the revised version on our website or within the Platform. Continued use after posting constitutes acceptance of the changes.
9. Governing Law
These Terms are governed by the laws of the State of New Mexico and applicable federal laws.
